Request Birth Certificate

Requesting Birth Certificates

There are two ways to request a copy of a birth certificate: by mail or in person. The State of Rhode Island charges a fee of $22 if obtained in person or $25 if requested by mail.

  • By mail: Birth certificates can be obtained by writing to the Office of Vital Records, 6 Harrington Road, Cranston, RI 02920 or at any city/town hall within the state of RI. If you send for a birth certificate through the mail, you must include a photocopy of your driver’s license or State ID.
  • In-person: Request may be made in person at any city/town hall within the state of RI or at the State’s office of Vital Records in Cranston.

Please be advised that in most cases, you will receive your records more quickly from the city or town locations than from the State Office.
